MINO OSKI AIN DAH YUNG — CASE MANAGER

SALARY:

DOQ. Full-time position with benefits. Monday-Friday, daytime and evening hours. Occasional weekends may be required. Flexibility required.

SUMMARY:

Case Manager will partner with the; Coordinated Entry System, Foster Care System, and larger community to assist eligible people gain access to the permanent supportive housing at Mino Oski Ain Dah Yung. Provide individualized case management, helping the residents access needed resources, support, and life skills to maintain housing stability and balance in all areas of life. Collaboratively develop an individual case plan with input from the resident and any other providers with which they are working. Assist clients with obtaining mainstream benefits. Solicit and coordinate interventions and services with partner or other identified agencies and organizations for residents accepted into the housing programs. Run groups and develop programing focused on learning life skills, mental health, healthy relationships, etc.

ACADEMIC REQUIREMENTS:

A bachelor’s Degree in Social Work, Child Psychology or related field preferred.

EXPERIENCE:

Must have at least 4 years of experience working within the field of Human Services, and at least two years direct care an/or case management experience in a community service agency. Candidates must be knowledgeable of the Native American Indian culture, values, and family systems.

ADDITIONAL REQUIREMENTS:

Must have valid MN Driver’s License and pass DHS Background Check.
