SALARY:
DOQ. Full-Time, non-exempt (40 hours per week).

SUMMARY:
The Street Outreach Case Manager will work in conjunction with the collaborative—
“StreetWorks,” through conducting Street-based outreach to homeless youth. Focus will be on
building strong networks to provide longer term Case Management to youth with a focus on Shelter,
housing and cultural connections. Half of the position will be dedicated to Street Outreach and Half
time to Case Management. This position will be responsible for coordinated entry, HMIS data entry
for Outreach and Case Management clients. Street outreach shifts will include some late nights and
weekends. Street Outreach Case Manager will be required to attend all Agency and Street Outreach
meetings/ trainings.

REQUIREMENTS
A minimum of a High School Diploma or General Education Diploma (GED).
The candidate will have to pass DHS Netstudy and Federal Level Background check.
Current Drivers license and car insurance.

EXPERIENCE
At least two years of experience working with youth in an educational or human service or homeless
youth nonprofit setting. Must have experience working with at-risk populations and the ability to
work constructively with youth who exhibit a wide array of challenging behaviors.
Knowledge of harm reduction, trauma-informed care and positive youth development is preferred.
